You drink alcohol
Alcohol could disrupt your sleep. Some people easily fall asleep when they have more than enough to drink. You are on medication

You take a hot shower before you sleep

You should take a shower several hours earlier than your time to sleep.
You exercise late
It is a good idea to exercise frequently, and it could positively impact your sleep. The problem is when you exercise late. You increase your heartbeat, and your energy level is up. As a result, calming yourself down and going to sleep could be a challenge. Exercise during the day and avoid strenuous exercises at night.
You keep using your devices
You will have a hard time sleeping when you use your computer and phone just hours before you sleep. Your brain receives a signal that it is not yet time to sleep because of blue light, and sleeping becomes a challenge for you.
The kind of bed you have is not suitable for you
